Counter Notification – Restoration of Material

  2. A description of the material removed and its original location on the Site.
  3. A statement, under penalty of perjury, affirming your good faith belief that the material was removed due to error or misidentification.
  4. Your name, address, and phone number, along with a statement consenting to the jurisdiction of the federal district court in your location (or any judicial district where the service provider is located if you reside outside the United States) and agreeing to accept service of process from the original claimant.
